Both organizations will work with hospital leaders from initial assessment through clinical implementation and performance review, helping each hospital determine which care can safely and appropriately remain local and what operating model is needed to support it. For rural hospital executives, the experience begins with the hospital's own data and priorities, not a predetermined technology or service bundle. The opportunity is both clinical and operational

Federal data list more than 1,700 Critical Access and Rural Community Hospitals across the United States and the need for sustainable rural care models is urgent. Since 2005, over 250 rural hospitals have closed and recent data presented through Becker's Healthcare Hospital Review estimates over 700 rural hospitals-approximately one-third of rural hospitals nationwide-are at risk of closing, with more than 260 at immediate risk (Stroudwater Associates Closed Hospital Report). Many rural hospitals already have telehealth technology, yet programs can remain underused when they lack clear ownership, standardized workflows, ongoing training, meaningful performance measures or the right clinical partner for each service line. Stroudwater's rural telehealth guidance emphasizes that technology alone does not guarantee impact; it must be integrated into clinical workflows, monitored through data and aligned with broader clinical and financial priorities. The collaboration addresses that missing middle by connecting strategy and economics with technology-agnostic clinical staffing, credentialing, implementation, training and ongoing performance management. A multi-network study of 4,324 tele-emergency cases across 65 rural hospitals found that telehealth intervention averted 20% of anticipated transfers; 43% of those patients were then routinely discharged from the local hospital, with average avoidable transport savings of $2,673 per averted transfer. The collaboration is not intended to prevent transfers when a higher level of care is needed, it is designed to help hospitals identify clinically appropriate opportunities to retain care, reduce delays in specialty decision-making and strengthen local teams.
